Each step builds on the last, requirements are accumulative.

Smooth dirt roads
Stock 2wd / 4wd vehicle
Ground clearance not an issue
Pretty much any vehicle capable of driving on a dirt road

Rated 1.5
Roughly graded dirt roads / Forest roads
2wd / 4wd, 4wd is recommended for poor weather application (i.e. snow/rain).
Ground clearance not usually an issue
Good set of aggressive tires recommended for poor weather

Rated 2.0
Unimproved or rarely graded road
4wd suggested but not necessarily required
Extra ground clearance may be needed
Good set a aggressive tires recommended

Rated 2.5
Rarely maintained roads and light trails
4wd highly suggested
Extra ground clearance suggested
Driver may find extra care and a bit of driving experience useful.

Rated 3.0   
Trails in difficult terrain and washes
